CQT Site Visit Reports give program administrators and funding agencies a detailed account of consumers’ honest feedback about the program and a clear list of individual issues to address.

Within ten days of the site visit, CQT furnishes the director of the program with a written report. The report includes:

No consumer names or identifying information are included in the written report. If an individual consumer gives permission for their name to be shared with facility staff to have a request or concern addressed, the consumer’s name is only shared verbally during the post-visit immediate report to staff.

For PRP site visits, the CQT Site Visit Report is also forwarded to the Core Service Agency (CSA) of that jurisdiction.

For inpatient facility site visits, the CQT Site Visit Report is forwarded to the CEO of the facility as well as MHA staff.

To protect the confidentiality of consumers, staff and programs, CQT Site Visit Reports are not public documents. A composite of consumers’ comments, requests and concerns is accessible via each year’s Annual Report.
